Care Manager Community Services
An immediate start is required for a highly experienced Care Manager who is compassionate and dedicated in case management.
RESPONSIBILITIES INCLUDE:
- Case management for the elderly.
- Development of Care Plans.
- Supporting clients within the Shepparton region.
REQUIREMENTS/EXPERIENCE:
- Sound Knowledge of the active models of the health sector.
- Tertiary Qualification in Social / Health Science.
- Current Police Check.
